Waiver / liability release

Waiver of Liability
In consideration of my participation of classes at Mat Club Martial Arts, LLC, I understand and acknowledge that I will be engaging in activities that involve risk of serious injury, including permanent disability and death, and severe social and economic losses which might result not only from my own actions, inactions or negligence, but the actions, inactions or negligence of others or the condition of the premises or of any equipment used. I further understand and acknowledge that that there may be other risks of participation that are not known and cannot be reasonably foreseen at this time. I hereby assume all the foregoing risks and accept personal responsibility for the damages following such injury, disability, or death.
Therefore, I do hereby relieve, release, absolve and hold harmless the Mat Club Martial Arts, LLC, its employees and agents of and against any liability or loss which may arise, should any
injury or damage be sustained by the named participant, or others, as a result of my participation. This release shall be binding on my legal representatives, heirs and assigns.
COVID 19 Waiver
I am choosing training at Mat Club Martial Arts, LLC. I understand that there are risks associated with my participation and I fully accept these and release Mat Club Martial Arts, LLC and its staff from any responsibility related to any likelihood of contracting COVID-19 during my training. I fully confirm that
-I have not tested positive for COVID-19 within the last 30 days
-I have not been in contact with any confirmed positive cases of Covid within the last 10 days
-I do not have any symptoms currently related to COVID-19.
